Area: Must be able to work in Indianapolis, IN.
Licensure Requirements: Must have open, active, and in good standing IN State Pharmacist License
Education: Pharm. D.
Experience Needed: 1+ years in a LTC setting
Schedule: Full-Time
Pay: $70.00
Background/ Drug Requirements: Ability to successfully complete background check, health and drug screen
Pharmacist
Job Responsibilities:
• Review and verify medication orders for accuracy, appropriateness, and potential drug interactions for long-term care facility residents
• Conduct comprehensive medication regimen reviews to optimize therapeutic outcomes and identify potential medication-related problems
• Collaborate with physicians, nurses, and other healthcare professionals to ensure safe and effective medication therapy
• Provide clinical consultation and recommendations regarding drug therapy, dosing, and medication management
• Monitor and document adverse drug reactions and take appropriate action to resolve medication-related issues
• Ensure compliance with federal and state regulations, including survey readiness and regulatory standards
• Participate in facility quality assurance programs and initiatives to improve medication safety
• Educate nursing staff and other healthcare team members on proper medication administration and pharmaceutical care
• Manage and oversee pharmacy services for one or more long-term care facilities
• Prepare and maintain accurate documentation of all clinical interventions and consultations
