Job Description:
Schedule: Mon - Fri 10am - 6:30pm and every forth Saturday/Sunday from 8am - 4:30pm (will have one day off before and after their working weekend)

Relocation Bonus: $1500 to Eligible Candidates
We are looking for a motivated pharmacist who has experience identifying and resolving drug-therapy problems by communicating with patients, caregivers, providers, and other members of the patient care team. The ideal candidate will be detail-oriented and understand the importance of ensuring the patient is set up for success post-discharge. The ideal candidate will have initiative, flexibility, and creativity. The ideal candidate should have knowledge of common disease states and knowledge of outpatient services that would benefit a patient. The ideal candidate should be open to learning new disease-states when necessary such as transplant. The ideal candidate should have at least one year of residency training and have board certification.
Complete Discharge Medication Reconciliations (DMRs)
• Ensure medication history and admission medication reconciliation are entered appropriately
• Resolve incorrect doses, outdated meds, and inappropriate therapy based on evidence-based guidelines by communicating with patients, caregivers, and providers
• Reconcile inpatient orders with provider recommendations and patient’s current health status to ensure discharge orders are appropriate
• Work with inpatient case management and outpatient pharmacy services to ensure patient has access to medications upon discharge
• Provide medication counseling including recommended drug monitoring and disease-specific goals when appropriate
• Triage and resolve issues that arise post-discharge when appropriate
Participate in Quality Improvement Initiatives
• Readmission reduction tracking
• Medication discrepancy resolution and interventions

• Pharmacy student and resident precepting
Educate and counsel patients, families, and healthcare team members on safe and effective medication use. Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, surveillance, and restricted-use programs. Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up. Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development. Identify, report, and help resolve medication safety events, adverse drug reactions, and errors. Demonstrate proficiency with EHRs, pharmacy informatics systems, and medication automation to optimize clinical decision support and documentation. Contribute to professional advancement through practice innovation, research, specialty alignment (e.g., critical care, oncology, infectious disease), committee participation, and scholarly activity. Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas. Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments. Other duties as assigned. Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
