In this hybrid role, you will spend approximately 60% of your time focusing on clinical responsibilities, particularly in Population Health and Ambulatory Care. You will actively manage medication regimens for patients, with a strong emphasis on target disease states such as diabetes. The remaining 40% of your time will involve staff pharmacist duties, providing a balanced mix of clinical and operational pharmacy work. There is potential for this role to transition to 100% clinical pharmacy as the program grows.
Responsibilities Include
- Collaborating with patients and healthcare providers to optimize medication therapy
- Managing clinical programs within the ambulatory care setting
- Supporting the expansion of clinical pharmacy services within the health system
- Balancing clinical and staff pharmacy duties in a hybrid role
Qualifications And Desired Experience
- Pharmacist licensed in Pennsylvania
- Experience in ambulatory care or inpatient pharmacy practice
- Completion of a residency program is highly preferred
- Strong commitment to patient-centered care and clinical program development
- Familiarity with Meditech pharmacy systems is beneficial
