Minimum Requirements:
- Must have State License to Apply
- EPIC experience
- Experience with medication reconciliation for discharge medication list
- Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
- Communication skills for discharge counseling with patients/families
- Clinical knowledge of antimicrobial stewardship, including interpretation of culture results for recommendations
- Clinical knowledge of UTI, pneumonia, and SSTI for recommendations
- Experience with pharmacokinetic dosing management/monitoring
- Experience with anticoagulation dosing management/monitoring
