We're hiring a licensed pharmacist for a contract position in Cleveland, OH at a hospital where you will use clinical expertise to ensure the safe and thoughtful distribution of medications to patients with varied medical needs. Responsibilities include maintaining drug inventory, preparing and dispensing medications, ensuring compliance with federal and state pharmacy laws, participating in formulary management, conducting medication reconciliation and discharge counseling, supporting antimicrobial stewardship and pharmacokinetic dosing, managing controlled substances, participating in quality assurance and P&T activities, providing education to staff and patients, and documenting pharmacy transactions. EPIC experience is required for medication reconciliation and related documentation.
Minimum Qualifications:
- Active state pharmacist license required.
- Experience with EPIC required.
- Experience with medication reconciliation for discharge medication lists required.
- Experience in preparing, sterilizing, and labeling parenteral medications and sterile products required.
- Experience ensuring controlled substance accountability and recordkeeping required.
- Experience with antimicrobial stewardship and interpretation of culture results required.
- Experience with pharmacokinetic and anticoagulation dosing management/monitoring required.
- Ability to perform clinical consultation and discharge counseling required.
- Minimum 6 months of pharmacist experience preferred.
Preferred Qualifications
- Previous hospital pharmacy experience preferred.
- Experience participating in Pharmacy and Therapeutics committees preferred.
Assignment Details
- Shift pattern: 5x8.
- Reports to the Director of Pharmacy under general supervision.
- May be required to participate in monthly inspections of pharmacy and patient care areas.
