We are seeking a Clinical Pharmacist for a 3-month assignment starting May 1st. This role offers a consistent weekday schedule and focuses on clinical pharmacy services in a home infusion or acute care setting, working closely with interdisciplinary healthcare teams.
Schedule: Monday – Friday (8:30 AM – 5:00 PM)
Term of Assignment: 3 months (Starting May 1st)
Job Duties:
- Evaluate patient eligibility for home care services and assess appropriateness of prescribed therapies
- Develop and maintain patient-specific care plans, including therapy goals and medication management
- Review lab results and perform pharmacokinetic dosing to optimize therapy
- Provide clinical dosing recommendations and document interventions
- Maintain accurate medication profiles and perform prescription order entry
- Ensure proper preparation, compatibility, and storage of medications
- Dispense medications and supplies while ensuring documentation, authorizations, and billing compliance
- Collaborate with physicians, nurses, and healthcare teams to support safe and effective patient care
Requirements:
- Active Washington Pharmacist license (clear and in good standing)
- Minimum of 2 years recent acute-care experience OR 1 year recent home infusion experience
- Strong knowledge of IV admixture services and intravenous therapy
- Ability to evaluate and monitor drug therapy for home care patients
- Strong collaboration and communication skills within a healthcare team
