Education: Pharm. D.
Experience Needed: 1+years of previous pharmacist experience.
Schedule: Full-Time
Pay: $114,400-$145,600 DOE
Background/ Drug Requirements: Ability to successfully complete background check, health and drug screen
Pharmacist
Job Responsibilities:
• Review and verify medication orders for long-term care facility residents to ensure accuracy and appropriateness
• Conduct comprehensive drug regimen reviews and identify potential medication-related issues
• Collaborate with physicians, nurses, and healthcare teams to optimize patient medication therapy
• Monitor for drug interactions, contraindications, and adverse effects
• Ensure compliance with federal and state regulations governing long-term care pharmacy operations
• Provide consultant pharmacist services and recommendations to facility staff
• Participate in interdisciplinary care planning meetings and provide clinical expertise
• Maintain accurate documentation and records in accordance with regulatory requirements
• Educate healthcare staff on proper medication administration and storage procedures
• Respond to medication-related inquiries and provide drug information support
• Oversee medication storage, handling, and disposal protocols within the facility
• Conduct quality assurance activities and participate in continuous improvement initiatives
