This clinical pharmacist opportunity is available for an experienced professional seeking a contract role in Vermont. Location: Williston, VT (with work at a nearby hospital in Berlin, VT)
Job Type: Contract (4 to 6 months)
Schedule
- Monday through Friday
- Shifts: 6:30 AM3:00 PM, 10:00 AM6:30 PM, or 12:00 PM8:00 PM
Qualifications And Experience
- Minimum of 5 years of clinical pharmacy experience
- Strong intravenous (IV) pharmacy skills
- Licensed in Vermont or able to obtain a provisional (temp) license quickly, with plans to secure permanent licensure during the contract
- Previous experience with medication reconciliation, order verification, vancomycin dosing and lab monitoring, and renal dosing
Responsibilities
- Perform medication reconciliation to ensure accurate patient medication histories
- Verify medication orders for safety and accuracy
- Manage dosing and lab monitoring for antibiotics, particularly vancomycin
- Adjust dosages based on renal function and collaborate with healthcare teams to optimize patient care
This role is designed for a pharmacist comfortable with traveling or relocating to Vermont. The state offers a temporary licensing option to facilitate swift onboarding, with support to transition to permanent licensure during the assignment.
