Job Details
All pharmacist positions at ExactCare Pharmacy are expected to deliver on quality standards that allow us to ensure proper medication profiles are managed, reviewed and verified. Our Pharmacists are considered a ‘Clinical Pharmacist’, meaning you will have the opportunity to perform comprehensive medication reviews and targeted interviews. Therefore, clinical knowledge of current industry standards is vital. The Clinical Pharmacist is responsible for adhering to company policies and procedures and expected to help build team capabilities through leadership skills and follow up.
Responsibilities
- Ensuring clinical accuracy and execution of patient prescriptions
- Verification of hard copy and digital Rx’s
- Communication with healthcare professionals and making sure that the Dr. call line is answered
- Supervision and execution of incoming and outgoing prescription transfers
- Perform DUR to identify medication therapy management eligible patients and identify clinical intervention per patient with follow up
- Ensure all legal, privacy and compliance requirements are met
- Other duties as appropriate

Education
- Bachelor’s degree in Pharmacy or Doctorate in Pharmacy (PharmD)
- Active state of Texas Pharmacist license in good standing
- Completion of a post-graduate year 1 residency (PGY1) and/or Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists
Experience
- Experience with medication therapy management, comprehensive medication reviews and targeted interviews
- Strong experience with Medication Therapy Management and comprehensive medication reviews
- Prior interface with OutcomesMTM and Mirixa, a plus
Skills & Abilities
- Bilingual in Spanish/English, a plus

Physicals/Mental Demands
This position is administrative in nature and will present physical demands requisite to a position requiring: hearing, seeing, sitting, and talking. Schedule
This is a part-time position with an expectation to work up to 29 hours per week and be available outside of normal business hours to meet customer expectations on an ad-hoc basis. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
